Instructions - br The Indiana name change for an adult as described in the State Code 34-28-2 is done by filing a petition to the county's civil court where you are a resident. It is easier, if you were just married, to have your certified marriage license with the raised seal or if you were recently divorced to have your divorce decree. Typically, you will not have a court hearing in Idaho but it depends on the circumstances of the name change.br br REQUIREMENTSbr Must be a resident for at least 6 monthsbr If just married, must have your original marriage certificate.br If just divorced, must have your divorce decree.br br Step 1 - Fill-Out the following forms:br Indiana Name Change Petitionbr Indiana Appearance by Self Represented Person in Civil Casebr Make sure all forms are NOTARIZED.br Step 2 - Make 2 copies of each.br br Step 3 - File the forms to the County Court Clerk's office in the County where your live (See County List Below). Pay the filing fee of $136.00 and you will be given a case number.br br Step 4 - You will need to fill out the Notice of Petition for Name Change and submit to the newspaper that serves in the County you reside. The ad must run for 3 weeks at least once per week and you must receive a Notice of Publication from the provider of this service.br The ad must run for at least 30 days before your hearing.br br Step 5 - After successful publication, wait until your court date and bring all the necessary documents with you. Make sure to bring the Indiana Order on Verified Petition for Change of Name, the Judge will authorize this form and you will be able to use it for changing your name at the DMV, passport agency, and any other private or public institution.
